What Is CCC Plus?
Were previously enrolled in Commonwealth Coordinated Care Plus.
Commonwealth Coordinated Care Plus (CCC Plus) is a statewide support program that offers long-term services and support to approximately 210,000 individuals with complex care needs across the Commonwealth of Virginia. Managed by Medicaid, this operates through an integrated delivery model and utilizes the full continuum of care, focusing on improving quality, access, and efficiency. Through CCC Plus, qualifying populations can benefit from care management and high-touch, person-centered program design.
Who Qualifies to Participate in CCC Plus?
CCC Plus is for Medicaid members who:
- Receive Medicare and full Medicaid benefits (dual eligible)
- Were previously enrolled in Commonwealth Coordinated Care (CCC)
- Receive Medicaid long-term services and supports (LTSS) in a facility or through one of the home and community-based (HCBS) waivers, except Alzheimer’s Assisted Living waiver
- Are eligible in the Aged, Blind, and Disabled (ABD) Medicaid coverage groups, including ABD individuals, previously enrolled in the Medallion 3.0 program
